Medicare SELECT Insurance Policy

Medicare SELECT is based on a network of hospitals. As a Medicare SELECT policyholder, if you receive care in a network hospital, you pay no Medicare Part A deductible. When a non-network hospital is used, you are responsible for payment of the Medicare Part A deductible.



Policy Form Series 130 and 131 availability will vary by state.

You are responsible for charges not covered by Medicare and this insurance plan. Please refer to the Outline of Coverage for additional information.

NOTE: If you choose the Medicare SELECT option, pre-existing conditions may not be covered for the first 90 days. Pre-existing condition means a medical condition that existed within the 90-day period before the effective date. Subject to limitations, which are explained in an outline of coverage for your state.

¹ Only certain hospitals are network providers under this policy. Check with your physician to determine if he or she has admitting privileges at the network hospital. If he or she does not, you may be required to use another physician at time of hospitalization or you will be required to pay for all expenses.
